Case Management
Case management services are available to older Vermonters and adults with disabilities through programs administered by the Division of Disabilities and Aging Services (DDAS). These services are funded under the Older Americans Act (OAA) and the Choices for Care Medicaid Waiver programs.
DDAS works closely with Vermont’s Area Agencies on Aging (AAA) and Home Health Agencies (HHA), the State’s case management service providers, to develop a comprehensive approach to the provision of case management services. These providers must meet our Case Management Standards to be approved to offer case management services. They must assure that staff hired to provide case management services pass a DDAS administered Case Management Certification Exam, prior to providing case management services to clients. Once a case manager is certified they must participate in at least twelve hours of continuing education each year to maintain their certification. DDAS sponsors an annual Case Management education and training program to help providers and certified staff meet the continuing education requirements.
Case managers are available to work with individuals, their families and caregivers to assist in finding services and resources that meet the individualized needs of each client. Designation as a ‘Certified Case Manager’ signifies the Case Manager possesses the knowledge, skills and experience required to provide appropriate services, as defined by DAIL's Division of Disabilities and Aging Services.

Agency Case Management Standards (December 2001)
Standards written to provide guidance for case managers and to describe acceptable case management performance. These standards apply to case management services provided to adults 60 years of age and older and to younger adults with disabilities through the Home and Community Based Medicaid Waiver, Enhanced Residential Care Waiver and Older Americans Act.

Case Management Certification Procedures (October 2002)
In order to ensure the statewide quality of case management services offered to these individuals, DAIL in partnership with Vermont’s Area Agencies on Aging and Home Health Agencies, has instituted a Case Management Certification Program. This certification program applies to individuals providing case management as a service of the Department’s Medicaid Waiver programs and for individuals providing case management as part of the services authorized under the Older Americans Act through the State Unit on Aging and the Area Agencies on Aging.

Case Management Action Plan
The purpose of a Case Management Action
Plan is to form a bridge from the assessment to the actual delivery of services.
The case manager develops the Action Plan with the individual receiving
services, caregiver, and/or family member. Issues and goals, as well as
plans/strategies for achieving the goals are worked out together. The Action
Plan provides a written summary of the issues/goals, plans/strategies,
responsible person(s), and target dates for completion of the
plans/strategies.
